Wingfield Skilled Nursing and Rehabilitation Center (“Wingfield”) is dedicated to excellence, integrity and ethical behavior as it provides health care services to its residents.

Wingfield does not discriminate and does not permit discrimination, including, without limitation, bullying, abuse or harassment, on the basis of actual or perceived race, color, religion, national origin, ancestry, age, gender, physical or mental disability, sexual orientation, gender identity or expression or HIV status, or based on association with another person on account of that person's actual or perceived race, color, religion, national origin, ancestry, age, gender, physical or mental disability, sexual orientation, gender identity or expression or HIV status.

If you feel that we are not doing our best to fulfill our mission, please let us know so that we can rectify the situation. We encourage anyone who has a relationship with Wingfield to contact us with any feedback, including any concerns or complaints about resident care, employee relations, discrimination, policy compliance, business ethics or any of the following:

  • Billing or reimbursement issues

  • Fraud/theft

  • Violation of governmental regulations

  • Workplace violence

  • Discrimination

  • Harassment

  • Conflict of interest

  • Substance abuse

The first step in resolving concerns or disagreements is to approach the community, team or individual directly, and our leadership at a local community is available to assist. However, if you prefer to remain anonymous, or your issues are not resolved, please contact our Ethics Hotline at (775) 583-8536 or fill out our Ethics Inquiry form at our parent company, which you can access here. You may also file a complaint with the Nevada Division of Public and Behavioral Health (DPBH) by calling (775) 684-4200.
